Forms for Certificate Students
Online Certificate Declaration Form
Form for students wishing to declare the Undergraduate Certificate in Global Health. Students declaring the certificate are expected to have read through all program requirements and policies, have a sense of what course or program they want to do for the field experience requirement, and be able to complete all coursework without extending their degree timeline.
Independent Field Experience Exception Request Form
Form for students wishing to propose an independent field experience for review and possible approval. Not all exception requests are granted, and students should carefully review the guidelines and policies on independent field experiences and meet with an advisor before submitting this form. Students should plan to submit this form two months before the start of the proposed independent field experience, and expect a 4-6 week turnaround on their request.
Certificate Course Exception Request Form
Form for students to submit a course exception request for a UW-Madison, study abroad, or transfer course. Study abroad courses with direct equivalencies to certificate courses already posted on the study abroad website do not require this form. It is important to note that curricular exceptions in the certificate are uncommon. Exception requests for Nutr Sci 203 are not allowed. Substitutions for core courses are allowed, but rare. Exceptions to elective course requirements must be upper-level courses (300+), and include coverage of global health topics evident in a course syllabus. Exceptions cannot be requested for topics courses. This form requires students to upload a complete syllabus for the proposed course, and provide a rationale for the exception request. Students should meet with their advisor before submitting an exception request, and can expect a 2-4 week turnaround on their request.
Forms for Major Students
Major Course Exception Request Form
Form for students to submit course requirement exception requests for either transfer, study abroad, or UW-Madison courses. This form is primarily used by students studying abroad or transfer students, as curricular exceptions are uncommon for other UW-Madison courses. Study abroad courses with direct equivalencies to major requirements already posted on the study abroad website do not require this form. Substitutions for core course requirements are allowed if appropriate, but the majority of exceptions granted are for depth courses. Exceptions to the capstone requirement are rare. Exceptions cannot be requested for topics courses. This form requires students to upload complete syllabi for the proposed courses, and provide a rationale for the exception request. Students should consult their advisor before submitting an exception request, and can expect a 2-4 week turnaround on their request.
